Neptune in the twelfth house
Neptune in the twelfth house is quietly at ease in the house of retreat. Solitude genuinely restores, and the inner life is rich: dreams remembered, atmospheres read, a sensitivity to suffering that operates whether or not it was invited. Work in hospitals, prisons, monasteries and other closed institutions turns up often, sometimes as staff and sometimes as visitor. What undoes a life here is escape in its various forms, and the difficulty of telling withdrawal apart from rest. The invitation is to build a regular practice for the inward hours, so that solitude is chosen and its material comes back out.

Which sign holds it
Under whole-sign houses the rising sign settles every other house at once. If you know your Ascendant, this is the sign your twelfth house holds and the planet that rules it.

Quadrant systems such as Placidus cut the houses by the angles instead, so a planet near a cusp can land in a different house from the whole-sign answer. The chart page shows both.
Reading it in context
A keynote is a starting point, not a verdict. What Neptune actually does in your twelfth house depends on its sign, its aspects, its dignity, and whether it is in sect. Two people with the same placement and different charts around it live it differently.
